Healthcare Insurance Enrollment, May 14, 2015
You can enroll your children in Medi-Cal or Healthy Kids healthcare insurance programs for children on Thursday, May 14th, from 9:00 – 11:00 am at the Blackford Elementary School Family Resource Center, 1970 Willow Street in San Jose. Please bring proof of address, proof of income and birth certificates for the children. Bring social security cards for the children if available.
